Any person using the facility must complete a personal information form and sign a liability waiver form, You will then receive an ID Card; this tells us you have completed all of the necessary forms to be able to use the facility. Minors must have the waiver form signed by a parent or legal guardian.
No one will be allowed to use the facility without these forms.
The information we receive will remain confidential and will only be used to contact your for matters pertaining to your membership or scheduled appointments, or to notify your designated contact in case of emergency, we respect your privacy and your information will NOT be shared.
Print and complete the appropriate registration forn along with a waiver of liability. Bring your completed forms to Game Day Training at 117 E. Las Vegas Street, Colorado Springs and receive your ID Card.
